* U- E- X5 D- }$ @0 Bfor neurological injury, with most treatments designed to limit secondary damage.
, f& X- x" j+ a1 @- h0 g8 ^The idea that exogenous stem and progenitor cells have the potential to- V3 C7 i& K( [2 _: J& E/ \& \
promote a reparative response to injury is now supported by pre-clinical data,3 k' M% p2 p5 c  v6 \! i
prompting early translational studies using progenitor cell therapies. To that end,
& f2 b5 B4 U! D% P+ mthis text explores the unique aspects of neurological injury and focuses on the critical
% D  e' K# R# j/ |1 V' P- |9 T- jtranslational issues of cell delivery: routes of administration, types of progenitor
& D& G3 j$ }0 e9 @. A4 `cells (alone and/or in combination), timing of delivery, and adjuncts to promote cell+ g7 c, y) p3 y. E1 F$ C. |
engraftment/survival/effectiveness. Finally, measuring the effects of transplanted
$ u# m5 @6 E4 `. Wcells and cell tracking is explored in several chapters.. p* H+ N) {5 V) g
Even while compiling this monograph, the paradigms of how cell-based therapeutics
: b) Z- ^+ L- g% r3 Y. R! O" aaffect neurological injury have been changing. Mechanisms that seemed/ w" k* d1 a5 X8 T. g1 h! c
reasonable and well-supported are being re-examined, and this necessarily drives9 N7 Y$ _8 T, h* a
the movement toward various clinical trial plans. The earliest clinical trials are5 d$ Y5 L" F0 x7 A% m3 l( q
beginning, and as this monograph goes to press, Phase 2 studies in TBI and stroke
. e! @: q. d. v& ~8 x/ }will begin. The developments in the field may ultimately offer realistic hope for# s: b, n4 H& m6 B  `
improvement in patients with severe injuries, and the work on these vexing problems
3 f* I4 Y, {+ G  ^8 ywill be the key to unlocking future treatments.
